Rear rack problem

As you may already know I've changed the rack on Ruby. This is the one I have now. It came with the bolts and indicator brackets + 4 washers.

But today I discovered that it seems to be loose where the bolt goes into the frame via the indicator hole. I did put a washer on the inside of the frame and tightened it as much as I could but I can wiggle it up and down so it won't be any good for taking any weight.
Is there supposed to be something else there?
The only thing I left off was the brackets for the indicators and the washer that would have gone there (I think), would that have made a difference?
PS. There is a sleeve that fits onto each bolt which I thought was to thread the wires through. I don't think that Nuttys rack has them.
