Sometimes the alloy washer gets a burr or ridge around the hole or outside edge due to it getting 'squished up' around the bolt head or into the hole when being tightened previously.
If the washer is there it's worth just making sure there are no burrs or ridges stopping it seating properly (or just fit a new one).
