Hi Newt,
Got some way toward the bottom of this... I have replaced the camchain & Cam sprocket as both were badly worn. I never noticed before, but I can confirm the cam chain length is a multiple of the sprocket, leading to uneven wear. There is still a rattle 
but it now runs properly (the wear was allowing the timing to wander).
It's definitely coming from the left side of the engine..
