2 ways of doing this: (that I use anyway)
1) use a cheepo flat earpiece that just plugs in (reliable and cheap).
2) use an A2DP stereo bluetooth earbud set from Hong kong eBay.
Using the latter with an iPhone allows music to be played and then the Sat Nav instructions over-rides the music and pipes up when instructions are needed. As the setup is basically a normal set of earbud's with a Bluetooth plug in unit I can also use my rather dapper Seinheiser noise cancelling 'phones with it too; the microphone is in the Bluetooth unit so it will accept any headphones and you can still use the 'phone.
Cookie has a fully bluetooth helmet and gets on fine with that setup using a Tom Tom Rider.
