The plastic spacer is probably why it worked loose!

Don't forget the swingarm bolt needs to be tight enough so the inner part of the bushes are tight against the frame and not at all moving with the swingarm.
All swingarm movement should be through the rubber bit twisting, not the bushes moving on the bolt.
